If asking for a quote, please let us know if you wish to have the questionaire sent as a Word file, a text file or posted to you as a hard copy. If you want a hard copy, please include your address.

Payment

If paying by cheque, we will provide the address to which the cheque should be sent when we supply the report.The facility to pay by Credit Card or via a PayPal account will be added shortly.
